Camp Shemesh Tuition (Based on eight-week enrollment)
 
Early bird member: $ 1750
Early bird non-member: $2010
Regular member: $ 1965
Regular non- member:$2240
Additional $100 mandatory security fee per child

Payment is due in full with this form. If you need to pay in installments, please contact our bookkeeper, Esterina Herman at eherman@aabjd.org to set up a payment plan.  Payment plans must be paid with credit card or e-check.  All payments must be made by June 1st.  Once a payment plan is in place she will give you a code to use that will allow you to fill out this form. 
Camp Lunch will be available for purchase starting the second week of camp.  You can choose to purchase everyday or just Friday pizza lunch.  The option to pay per week is available for those who will be away for some weeks of camp.  If you are ordering camp lunch, you must order for every week your child is in Camp.
In the event that my child requires medical care (and the determination thereof shall rest solely with you if you are unable to reach me I, authorize any doctor or doctors and/or hospital to which my child may be brought to take and perform all necessary procedures and render any indicated treatment, including the administration of anesthesia if needed, and the performance of an operation if in the opinion of said doctor or doctors the same is necessary, while such child is under the jurisdiction of Camp Shemesh of Congregation AABJ&D.
I, grant permission for my child’s picture to appear on the synagogue’s website, and other materials prepared for, by or on behalf of Congregation AABJ&D to advertise or promote its Nursery School, Summer Camp and/or Youth Department Activities.

I hereby allow my child  to take local walks with a staff member.
